Continuing Services Agreement 

authorize PUC Distribution Inc. and the Public Utilities Commission of the City of Sault Ste. Marie to keep the utilities on in my name at the above address when a tenant has given final notice and before a new tenant will be taking over providing that my account is in good standing as outlined in our “Conditions of Service”.   This will include monthly basic service charges plus any consumption on the meter(s). The standard account setup fee will not apply for the transfer of an account to the Owner.   Bill me at the address as listed below:

In the event that I choose to terminate the service(s), I understand that I must request it in writing and PUC shall not be liable for any damage that might arise from this action (in accordance with PUC’s Conditions of Service).  The standard account setup fee will apply when the service is reconnected.
